Risk Assessment Questionnaire
This risk assessment is designed for people 12 to 25 years old without known heart conditions. It uses information from the American Heart Association’s 14-step screening process to predict risk for long-term health as well as sudden death from congenital and genetic heart conditions in youth. Point values are assigned based on the severity of the condition and the total number of conditions checked. This risk score will not adequately reflect the long-term or lifetime risk of death but is meant to determine the importance of a comprehensive heart assessment for your child.
Risk categories are based on the overall score generated from the answers provided. There are many factors that can lead to heart conditions that are not necessarily measured in this assessment including obesity, smoking, and cholesterol levels.
